FAQ-How can I obtain my transcript?

How can I obtain an official or unofficial transcript?

1)         To order a copy of your transcript, complete the Transcript Request Form and mail or fax it to Mercy High School.

2)         Students/Alumnae can request official transcripts of their academic records by writing to Mercy High School Guidance Department * 1740 Randolph Road * Middletown, CT 06457.  Requests can also be faxed (860) 704-2346 to the Guidance Department with or without using a form.

All written request must include:

·        full name (including any other names you may have used while attending)

·        date of birth

·        dates of attendance

·        complete and accurate addresses of transcript recipients including Zip codes, and the requester’s address and telephone number in the event that there is a problem with the request

·        a daytime telephone number in case of problem

·        your signature

Requests must be signed even if they are faxed.  There is a $3.00 charge for transcripts.  Request forms can also be completed at Mercy High School in the Guidance Department. Since we need your signature, we cannot accept phone requests. 

Can I have a transcript mailed to my home and/or to myself?

Students can request that their transcripts be sent to themselves.  Note, however, that such transcripts are stamped “unofficial”.  Students are cautioned that some recipients will not accept transcripts that have not been sent directly from the school.

What could restrict getting a transcript?

Official transcripts may be withheld by appropriate High School officials if some financial or other obligation to the High School remains unmet.  Restrictions can be made by:

·        Registrar’s Office

·        Business Office
