MERCY HIGH SCHOOL

IMPORTANT MEDICAL INFORMATION

 

State law and C.G.S.  Secs. 10-204a and 10-206 and the Middletown Board of Education require a health assessment by a legally qualified practitioner of medicine for grades K, 6 and 9.

Every student in 9th grade at Mercy High School is required to obtain a physical examination performed by her private physician during the 9th grade.  This can be done at her yearly exam when due.  PLEASE NOTEThis physical exam is mandated by the State of Connecticut for entrance into the 10th grade.

 

SPORT REQUIREMENTS FOR ALL CLASSES:

ALL PHYSICALS (MANDATED blue form or sport physicals done at walk ins/Urgent Care) are valid for sport clearance for 13 months from the date of exam and should carry through the entire season per the policies of the CIAC and the Middletown Public Schools.  A valid physical form must be on file with the nurse for clearance for tryouts.  

The blue, State of Connecticut health assessment record includes a section for sports clearance.  If the student goes to her private physician for a sport physical, this is the form to be used.    This form must be completed and on file in the Mercy High School Health Office prior to tryouts.
