MERCY HIGH SCHOOL
IMPORTANT MEDICAL INFORMATION
2018-2019 SCHOOL YEAR
State law and C.G.S. Secs. 10-204a and 10-206 and the Middletown Board of Education require a health assessment by a legally qualified practitioner of medicine for grades K, 6 and 9.
Every student in 9th grade at Mercy High School is required to obtain a physical examination performed by her private physician during the 9th grade. This can be done at her yearly exam when due. PLEASE NOTE: This physical exam in mandated by the State of Connecticut for entrance into the 10th grade.
Since many parents schedule appointments during the summer months, we are providing you with the proper form to be completed by your private physician. Please make an appointment with your doctor, complete your portion of the enclosed blue state form, and bring it to your scheduled appointment. The completed form must be returned to the school nurse while your daughter is in 9th grade.
FRESHWOMAN, SOPHOMORE, JUNIOR & SENIOR SPORT REQUIREMENTS:
The policy requires all athletes to have a current physical on file with the nurse prior to trying out for any team. Physicals are valid for 13 months from the date of the exam. If the physical will expire during the participant’s season, a new physical is required prior to the athlete trying out, practicing or playing in games.
It is highly recommended to obtain the physical by your private physician.
The blue, State of Connecticut health assessment record includes a section for sports clearance. If the student goes to her private physician for a sport physical, this is the form to be used. Either form must be completed and on file in the Mercy High School Health Office prior to tryouts.

The sports pre-participation form (yellow) is for SPORTS ONLY and does not fulfill the requirements for a complete mandated physical as required in 9th grade to start 10th grade.
